If your insurance carrier allows self-referrals, you may contact St. Francis Orthopaedic Institute directly for appointments without a referral from another doctor. To schedule an appointment with one of our physicians, please have your current insurance card, social security number and basic demographic information available.

If your insurance carrier requires a referral from your primary care physician, please schedule your appointment through your primary care physician.
If you have an injury that occurred at work and will be filing a Workers' Comp claim, you must first notify your employer of the accident. There is a specific guideline that must be followed in order to have the injury covered under Workers' Compensation. Your employer must call (706) 324-5804 to schedule the appointment and furnish the necessary insurance information.

All previous X-rays or studies must be brought to your appointment. Please contact any other treating physicians so that they may promptly fax any pertinent medical records to our office.
You also need to bring your insurance information and any appropriate referrals. If you are a new patient filing under Worker's Compensation, please have your adjuster or employer contact our office to set up your appointment.
